Allergen Matrix Template
A menu-item-by-allergen grid for the nine major food allergens recognized in the U.S.: milk, eggs, fish, crustacean shellfish, tree nuts, peanuts, wheat, soybeans and sesame (sesame added by the FASTER Act, effective January 1, 2023). Mark X for "contains" and M for "may contain / cross-contact".
You fill in your own menu from your ingredient labels and sub-recipes - the template doesn't know your recipes and can't make any dish "allergen-free".
Not legal advice. These are blank templates. Values are cited from the U.S. FDA Food Code 2022, a model code that states and counties adopt with changes. Follow your local health department - its rules take priority.

Common questions
Will these logs make me pass a health inspection?
No template can promise that. They help you keep steady records. Rules vary by state and county, and your local health department's rules come first.
Where do the temperatures come from?
The U.S. FDA Food Code 2022, with section numbers printed on each log. Your area may use an older edition or different values.
How do I get the files?
Right after payment you see download links, and they are emailed to you. Links work for 7 days, so save the files. Print as many copies as you need for your own business.
Is this a subscription?
No. It is a one-time purchase.
